Objectives

After reviewing the module and completing the accompanying activities, you should be able to:

  • Define transition assessment
  • Understand the different types of assessment
  • Demonstrate how a transition IEP is based on transition assessment and addresses any gaps
  • Write an assessment summary using a “transition lens”

You have learned the standard definition of transition assessment and different types of transition assessments. You have also been provided specific examples of formal and informal assessments.

  • CSAP/CSAPA
  • District assessment
  • Situational assessment
  • Self-determination evaluations

You have learned how to link or thread transition assessment information throughout the transition IEP. You have also learned how to determine if a gap exists between a student’s current skills and PSGs.

  • Jenny’s course of study or schedule of classes was determined using assessment data and consideration of how the classes would increase her likelihood of mastering her PSGs.
  • Jenny’s transition services were determined based on the activities that would assist Jenny to achieve her PSGs based on her current performance.
  • Jenny’s goals and objectives were determined by looking at her current performance and the performance level that she needed to achieve her chosen PSGs.

Finally, you completed a transition planning form on Tyler, or a student of your choice, which provided you with practice using transition assessment information to drive the creation of a transition IEP.

Now you have the knowledge and tools to create a data driven, comprehensive, and realistic transition plan for students.
