Accessible Transition From Part C to Part B Infographic

Your Service Coordinator will work with you on a transition plan to ensure your child receives supports and services base on their needs after Early Intervention (EI) ends. Early Intervention ends by your child's third birthday, unless qualifying for the Extended Part C Option.

2 years

Start to Plan

You and your service coordinator will start to plan and discuss options about what happens as your child approaches their third birthday.


2 years, 3 months to 2 years, 6 months

Notification is Made to the School District or BOCES
(Board of Cooperative Education Services)

With your permission, your service coordinator will send additional information about your child to the school district or BOCES where you live.


2 years, 3 months to 2 years, 9 months

Meet with your service coordinator and the school district or BOCES to talk about special education in preschool.

Interested in Special Education Services in Preschool?


2 years, 9 months to 3 years

If you are not interested in special education services in preschool, you will discuss other options to continue learning in your home, community and/or preschool without special education services.

If you are interested in special education services in preschool for your child, your school district or BOCES will schedule a screening and/or evaluation.


Prior to 3 years

EI services will continue. If your child is not eligible for special education services, you will discuss other options to continue learning in your home, community and/or preschool without special education services.

If your child is eligible for special education, you and the special education staff staff will develop an Individualized Education Program (IEP) to support your child in preschool.


By age 3

Your child will exit EI Services unless they meet the requirements for the Extended Part C Option.
